Therapists work individually with each child to target communication, behavior, and daily living goals. Sessions are customized to address unique strengths and challenges. This focused approach ensures consistent progress and meaningful skill development.

SOCIAL SKILLS GROUP ACTIVITIES

Children participate in structured group sessions that promote sharing, teamwork, and conversational skills. These activities help children practice peer interaction in safe, guided settings. The goal is to prepare children for school and community engagement.

    ASSESSMENT

    We perform comprehensive assessments which include the client's medical history, cognitive abilities, functional and emotional status, strengths, living environment, finances, and existing social support.

    CARE PLANNING

    The Care Plan is tailored to include recommendations regarding medical and health care issues, community resources, housing options, financial and legal planning.

    MONITORING

    Our Care managers monitor the progress and milestones of the plan to ensure that it continues to meet the person’s goals. Care Plans are modified as necessary to meet those goals.

    COORDINATION OF CARE

    Coordination of services may include home health agencies, homemaker and companion services, adult day care, special transportation or assistive technology.

    ADVOCACY

    We act as advocates for our clients and their family with physicians, insurance, hospitals, nursing homes, home care services and retirement communities.
  • 6

    HOME CARE SUPPORT

    For our clients, we coordinate the access, implementation, and coordination of services in a home setting. We arrange for needed in-home services, service modifications and needed equipment.
